If you are a health care professional and would like to refer a patient to a Marianjoy location for medical rehabilitation, please choose from the options below:
For Inpatient Admissions and Case Management call 630-909-8920
To make a referral to Marianjoy Rehabilitation Hospital, we request the following information:
- Face sheet
- History and physical as well as Physical Medicine and Rehabilitation consult, if available
- Therapy evaluation and notes
- Labs and vitals from the last 72 hours
- Medication list
